Welcome to the State of Utah's online application for the following public service programs:

• Medical Assistance - Medicaid, CHIP, UPP, PCN

If you need help completing the application, please click on the Online Help Icon. Our staff are available Monday through Thursday, 7:00 am until 6:00 pm to assist you.

The application tool will ask questions about you and all of the members of the household. Before you get started, you may want to get the following information ready:

• Income for all members in the home
• Social Security Numbers and dates of birth
• Assets information such as bank accounts, vehicles, homes, property
• Housing and utility expense information

Before you begin PLEASE temporarily disable your pop-up blocker using your browser settings. If desired, you may leave your pop-up blocker enabled and hold down the CTRL button when you click on either buttons. You may also add this website as a trusted site.

If you close out your browser at any time during the application process before you complete the screens, your information will not be saved and you will need to start over from the beginning.
